TURN IN ELECTRONICALLY (PREFERRED)


Pay all calculated registration fees using the PayPal Button below. Be sure to include PayPal receipt as Proof of Payment. Email all forms in pdf format to sports@myboysandgirlsclub.com. You will receive a confirmation email within 2 business days either congratulating you on your successful, or informing you of the forms/documents still missing. 


DROP OFF / WALK IN


Bring all forms, documents, and payments to the Northwest Boys & Girls Club located at 2661 NW 51st Street in Gainesville Monday through Friday between the hours of 10:00am and 5:00pm. Payments can be in the form of cach, check, or credit card (Visa or MasterCard). Email us at sports@myboysandgirlsclub.com 


TURN IN TO HEAD COACH (LEAST PREFERRED)


Head Coaches have discretion to collect forms and payments. The Head Coach may not want to be responsible for payments, especially cash. Coaches may ask that registration and payment be done via the other ways listed above. Please check with your team's Head Coach in advance of any deadlines.


REQUIRED FORMS AND DOCUMENTS


The following forms and documents are required before participation in tackle football. Click on the items in blue to download a pdf version of the forms. The documents not in blue you will need to collect to turn in.

  5. proof of date of birth - copy of birth certificate or passport

  8. preseason academic report (ending grade report from previous academic year)


If submitting electronically, email pdf versions of all documents and forms to sports@myboysandgirlsclub.com. You will receive a confirmation reply within 2 business days.

Click on the ADD TO CART buttons next to the Registration Fees that apply. Your fees will added to an electronic shopping cart and you will will be redirected to a secure PayPal payment system. After completion of your payment, please remember to print your payment receipt to include in your Registration documents.


REGISTRATION FEES

  1. $150.00 - Members registration fee
    1. you are considered a member if you have already registered for the upcoming academic year at one of our clubs in a non-athletic afterschool program.
  2. $175.00 - Non-Member Registration Fee
    1. You are a non-member if you have not registered FOR THE UPCOMING ACADEMIC year at one of our clubs in a non-athletic afterschool program.
  3. $50.00 - equipment rental fee
    1. Pay this fee only if you need to rent a helmet and / or shoulder pads. there will be an additional form required that will be provided at the time you pick up the equipment. equipment rentals are limited to the first 100 players that pay the equipment rental fee.
